- Hazen enlisted as a private in Company A, 3rd Regiment in the infantry of the Vermont National Guard for three years. He mustered in on 16 Jul 1861 at St. Johnsbury, Vermont. He died at Camp Griffin of disease in 1862. The reference says he died at Camp Griffin, "Va.", but that is most likely supposed to be "Vt."
