He received his B.A. from Oxford on 13 May 1737 and his M.A. on 11 Jul 1740. He became a Fellow of the College and in Apr 1741, he was nominated as one of the Radcliffe Travelling Fellows, on which University foundation he continued until 1751.

On 24 Jul 1751, he was appointed Joint-Physician of Bethlehem and Bridewell Hospital in London, as an assistant to his father, whose health was declining. When his father died the following year, he was appointed his successor.

John and his first wife, Elizabeth Culling, were married by John's brother, Rev. Thomas Monro, B.D.

John was a Candidate of the College of Physicians on 25 Jun 1752, and a Fellow on 25 Jun 1753. He was Censor in 1754, 1759, 1763, 1768, 1778, and 1785, and delivered the Harveian Lecture in 1757. He restricted the practice of his profession almost exclusively to insanity, in the treatment of which he was considered to have attained a greater eminence and success than any of his contemporaries.

In Jan 1783, he had an attack of paralysis, after which he gradually retired from his practice.

He lived for a long time at 53 Bedford Square, but in the beginning of 1791, he took up residence at Hadley, Near Barnet, County Middlesex, where he died.
